AppleCare for-Phone 4S

I  purchased and I-Phone 4S in  USA on early Feb 2012. I recall  also purchasing  at the same time the Applecare  World wide  for something like 100$

The  problem is  that I do not have any evidence for my  pruchase - Is there a way  to retrieve the contaract ?

Can I repair  my  I-phone in Israel  ( Under AppleCare coverage) ?  Where ?
