TS1630 How contact apple to report faulty iPhone 5 and get replacement

I bought and iphone 5 a little over 2 weeks ago and had a faulty left speaker and all the headphones and earphones left side doesn't work I got my phone " fixed" but they sent me a new one and I received it with the exact same faults I need a full iPhone 5 16gb white model replacement I paid way to much to have a couple of faults on this fault please help me I just want something I deserve as I paid the full amount not half someone please help me get through to apple I want a brand new iPhone 5 16gb white model in the box not in bubble wrap again PLEASE HELP!
